1. Voluntary participation and risks. AfroFusion Flow involves dance, stretching, aerobic movement, balance and coordination. Risks include strains, sprains, falls, collisions, fatigue and other injuries. I voluntarily participate, or authorize my minor child to participate, knowing these risks.
2. Health and personal responsibility. Participants should work within their abilities, take breaks, and tell instructors about relevant limitations or accommodation requests. Sankofa Travels with Kofi does not provide medical evaluations.
3. Liability. To the fullest extent permitted by applicable Virginia law, I release Sankofa Travels with Kofi and its directors, officers, employees, instructors and volunteers from claims arising from ordinary risks of participation, including ordinary negligence only where such a release is legally enforceable. This does not waive rights that cannot legally be waived, including claims for gross negligence or intentional misconduct.
4. Medical emergencies. Staff may contact emergency medical services when reasonably necessary. If I am the parent/legal guardian of a minor, I authorize staff to seek emergency assistance for that child when I cannot promptly be reached. I understand medical expenses remain my responsibility to the extent permitted by law.
5. Minors. Only a parent or legal guardian can authorize a minor's participation. Parental authorization does not waive rights that Virginia law does not permit a parent to waive on the child's behalf.
6. Conduct. Participants must follow reasonable safety instructions and treat others respectfully. Participation may be limited for safety reasons.
7. Acknowledgment. By checking the participant-specific box, the signer confirms they read this agreement, understand the inherent risks, and voluntarily participate or authorize their minor child. Each adult must personally agree; a purchaser cannot sign for an adult friend.
